Vendor Information
| In order to be set up as a vendor with Progressive Management, Inc we will need the following: |

- A Completed W9 form (click here to download)
- A Certificate of Insurance including Liability and Workers Comp, showing the insured as the Contractor’s name, with effective policy dates covering the period the Contractor worked for the Association. The Certificate must list the Association and Progressive Management, Inc as additional insured, and the Certificate Holder must list the Association, in care of Progressive Management, Inc, 4151 Woodlands Parkway, Palm Harbor, FL 34685.
